Volunteer Vaccine Form

In October 2021, it was decided by Administration that all of our volunteers be vaccinated for when they interact with the children while at school. Anyone who volunteers for a PTA event at the school needs to fill out our vaccination form which can be found below as well as on our website and app. This information will be shared with the school nurse only as necessary.
